Exogenous lipoid pneumonia

Large droplets of lipoid material in alveolar and interstitial macrophages are characteristic. Fibrosis and a granulomatous response typically occur.

Endogenous lipoid pneumonia

Finely vacuolated alveolar macrophages fill alveolar spaces when clearance is obstructed as around tumors, in COP (BOOP), or in amiodarone toxicity.
