Further history obtained after review of the biopsy: As a child, the patient lived in rural Mexico in a small cabin. The kitchen consisted of a small room with an open fire surrounded by a brick border. Venting was supplied by openings in the mud brick walls. Fuel consisted of corn cobs, wood, paper, or dried cow dung. The patient also made pottery--gathering the clay, beating it, sifting it (which was very dusty) and molding it into water jugs, etc. She did this occupation for approximately 15 years before her family moved to Mexico City. What is the diagnosis?
